Your first visit to our office establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor.
What to Expect at Your First Visit
To speed up your first visit and make “day one” with us more enjoyable, we encourage new patients to fill out our New Patient Forms below. Everyone should download the New Patient file along with either the ADULT or CHILD Medical History Form (dependent children of any age should use the CHILD Medical History Form)
