Due to the changed regulations regarding COVID-19 transmission, Bridge Medical Center now requires patients and their companions to complete a COVID 19 self-screening form before any appointment can be made.
In order to minimize the exposure to and transmission of the virus, we require that patient comes alone, unless assistance is required with mobility or communication, or if the patient is a minor. We are accepting same-day appointments.
As far as new patients, they must submit completed New Patients Forms and copies of their medical insurance coverage.
